What is a Fertility Awareness Check?

Fertility Awareness Check is the most predictive fertility test designed to provide an overview of your reproductive potential. The check can test eggs or sperm. Egg testing includes two hormone tests and a vaginal ultrasound, done on the first few days of a menstrual cycle, to measure the ovarian reserve and current supply of eggs. A semen analysis will indicate the health and quality of sperm.

*Fertility Awareness Check is $99 for egg or sperm testing. Partner testing is billed separately. Fertility Awareness Check is valid for new patients only and will not be submitted through insurance. The $99 fee will be credited back on future fertility treatment at FCI for new patients only and is not valid towards a doctor consultation or prior treatments. Only two Fertility Awareness appointments can be scheduled in a lifetime, and they must be at least 12 months apart.

Instructions for Scheduling An Appointment


Testing can be done Monday through Friday at any Fertility Centers of Illinois location.

  • The vaginal ultrasound and blood work needs to be performed between days 2-4 of your next menstrual period.  Please call us on day 1 of your menstrual period (first day of full flow) to schedule this appointment.
  • If you get your period during the weekend, please call us on the morning of the next business day to schedule your appointment.

Specimens for Semen Analysis (SA) Procedures

Monday-Friday ONLY: SA appointments are scheduled at all Fertility Centers of Illinois offices during the morning. Please contact the office for specific times.

Presenting the Specimen to the Laboratory: A specimen will be accepted from the male who produced it, or from his partner. If the male who produced the specimen is not personally submitting it to the lab, he must provide his partner with a copy of his photo I.D. and the completed and signed requisition form.

Instructions for Producing A Semen Specimen


It is recommended that you refrain from sexual activity for 2-5 days before producing the specimen for your appointment.

We encourage you to produce your specimen at home, using a kit that can be provided to you at any Fertility Centers of Illinois office. The kit contains a specimen container, label requisition form, and instructions. If you prefer to produce the specimen in our office, we provide rooms for this purpose.

  • Specimens should be produced no more than two hours prior to your scheduled appointment time
  • Specimens should be provided by masturbation only, into the specimen cup provided
  • Hands should be washed and dried thoroughly prior to producing a specimen
  • Do not use lubricants, creams, or lotions, as they are toxic to sperm
  • After the specimen has been produced, close the lid tightly
  • Complete the requisition form and label and affix the completed label to the specimen container
  • Keep the specimen container close to the body during transportation to maintain proper temperature
  • Submit the labeled specimen container to the laboratory technician at your scheduled appointment time, along with the completed and signed requisition for and picture I.D.

The person who produces the specimen must be able to provide picture identification and verify all label information with the laboratory staff according to laboratory standards.
